Abstract
진행성 반안면 위축증 (progressive hemifacial atrophy, PHA) 은 원인 미상의 후천성 편측 안모의 위축을 말하며, 주로 전두부 하방으로 5번 뇌신경인 삼차신경의 지배를 받는 부분에서 발생하며, 결체조직, 지방, 근육 및 연골골성 조직까지 영향을 미쳐 편측으로 위축된 안모를 보이게 된다. Parry, Romberg 등에 의해 처음으로 보고되어 Parry-Romberg syndrome (PRS)이라고도 불리며, 이후 Progressive hemifacial atrophy (PHA), Idiopathic hemifacial atrophy, Progressive facial hemiatrophy, Romberg’s syndrome 등의 다양한 명칭으로도 지칭되었다. 상기 질환에 대하여 외상, 자가면역, 감염 등 다양한 요인들이 원인으로 제시되나, 아직까지는 진단을 위한 표준화된 기준이 부족한 상태이다. 저자들은 면역억제제를 복용하는 환자에서 상악동 골이식술 및 임플란트 시술 후 발생한 진행성 반안면 위축증 증례를 경험하여 이를 보고한다.
반안면 위축증의 원인은 명확히 알 수 없는 상태이며, 특히 면역억제제를 복용하는 환자에게는 임플란트 식립 뿐만 아니라 발치나 신경치료 등의 치과치료를 시행하기 전 상기 질환에 대한 발생 가능성에 대해 주지시키는 것이 필요하리라 사료된다.
Progressive hemifacial atrophy (PHA) is a rare disorder characterized by acquired unilateral facial atrophy. This disease mainly affects the skin and the craniofacial tissue below forehead. It also involves dermatomes of the fifth cranial nerve. Atrophy of subcutaneous tissue, fat, muscle and osseocartilaginous structures were seen. It resulted in a sunken hemifacial appearance. Although epidermal cutaneous tissue was minimally involved, the tongue, gingiva, teeth, and palate may also be affected. The purpose of this study is to report a case of progressive hemifacial atrophy in a patient taking immunosuppressive drugs after implant and sinus graft surgery.
